When answering the questions, please start all of your answers on a new line directly below the question. If possible, please add two empty lines after each answer. Once completed, please save the document with a new file name, use the Bride's last name then the Groom's last name then the wedding date separated by a dash. Please be sure to save the final document as a ".doc" file.

Example: Lisa Smith & John Doe Wedding Date: October 10, 2010
"Save as" smith-doe-october-10-2010.doc
